Community Nursing

Boort District Health District Nursing Service is a service that can be accessed by you in your home.

Whether you’re an individual needing medical care at home, a GP with a patient who could benefit from home care or a patient transitioning from hospital to home, we are by your side.  We have a very clear objective to provide the very best of care, where you need it, when you need it, close to or at home.

Our nurses will visit you at home or at an agreed location suitable to your care needs.  The services you need are reviewed regularly to ensure that appropriate care is being provided always.  Three monthly reassessment and care plan reviews occur or as required and we strongly encourage client and or client representatives participation.

Boort District Health all also offer specialist advice on diabetes, asthma and wound management as well as providing health promotion and information sessions to the community.

The District Nursing service provides a range of services to the individual in their home that includes, wound management, medication management, general nursing support, palliative care, nursing care following hospital discharge, and assessment and referral to other service providers.

Our purpose is to:

  • Help people stay in their home and local community.
  • Provide referrals to other relevant services and care.
  • Promote health and well-being.
  • Recognise people’s rights to maintain their own independence and individuality.
  • Provide support to people so they can make their own informed decisions regarding their health and well-being.
  • Work with healthcare providers and services to provide a collaborative approach to health care.

You can access these services by contacting Boort District Health directly or by referral from your treating physician.

What is the Cost?

Charges apply as set by the Department of Health and an account is sent out from the Hospital at the end of each month. This service is provided five days per week Monday to Friday and excludes public holidays.
