Acute Care
The 7-bed unit at Boort District Health is designed to care for patients with acute or short-term medical conditions, to provide step-down care following hospitalisation at a larger regional or metropolitan service, to provide respite and end of life or palliative care. The beds incorporate transition care program (TCP) as well. We offer comprehensive onsite/Virtual and Contratced services, including care from a visiting medical officer, consulting clinicians such as Virtual ED, geriatricians, and a dedicated team of Registered Nurses, Enrolled Nurses, and Allied Health Professionals. Each patient room is equipped with its own ensuite for privacy and comfort. A private lounge area is also available for patients and visitors to relax and enjoy tea or coffee.

Transition care Program (TCP)
Boort District Health offers a Transition Care Program (TCP) aimed at supporting patients in their recovery after being hospitalised. This initiative is supported by the Commonwealth Department of Health and Aged Care, the Aged Care Quality and Safety Commission, and the Victorian Department of Health. Designed to span up to 12 weeks (84 days), TCP facilitates the consumer’s recovery either at home or within the hospital environment. By focusing on restoring functional independence and fostering confidence, our TCP delivers a diverse range of services, ensuring a comprehensive approach to rehabilitation.
