Boort District Health District Nursing Service is a service that can be accessed by you in your home, we are here for everybody. Whether you’re an individual needing medical care at home, a GP with a patient who could benefit from home care or a patient transitioning from hospital to home, we are by your side. We have a very clear objective to provide the very best of care, which is directed by you.
A Registered Nurses will visit you at home on arrangement. Reassessment and service provision are reviewed regularly to ensure that appropriate care is being provided to the client and their families/carers. Three monthly reassessment and care plan review should occur or as need arises with client participation and consent.
Boort District Health all also offer specialist advice on diabetes and asthma management as well as providing health promotion and information sessions to the community.The District Nursing service provides a range of services to the individual in their home that includes, wound management, medication management, nursing support and maintenance to the highly dependent and/or frail and aged person, palliative care nursing, nursing care following hospital discharge, and assessment and referral to other service providers.

You can access these services through Boort District Health and family, friends. Or an individual or a doctor can make the referral.
Charges apply as set by the Department of Human Services and an account is sent out from the Hospital at the end of each month. This service is provided five days per week.
